Course Overview

Incidents, Accidents & Failures – a review to inform design processes – good and bad.
This course is designed for young professionals in dam engineering and in particular Supervising Engineers or those aspiring to be Supervising Engineers under the Reservoirs Act 1975. It would also benefit those younger engineers providing design input on reservoir schemes and those providing supervision to reservoir remedial work schemes.

It will review a number of well-known incidents, accidents and failures as well as a number of less well known case histories, and also recent designs offered to fulfil matters in the interests of safety at sites.

In exploring these cases and the reasons for the problems occurring, the lectures will bring out examples of good and bad design practice and offer advice to prevent similar occurrences.

A selection of case histories will explore the remedial works adopted at a number of sites and how they were selected and review the appropriateness of the designs.

Part of the course will review methods used to meet the requirements of the MITIOS items.

Remedial works described will include grouting; diaphragm wall; filters; core raising and slope stability improvements; valve selection and replacement including a specialised lecture on valves; valve maintenance and repairs. The introduction of syphons and spillway design and improvement and review some innovative methods and materials for use in dams will also be presented.

Via discussion sessions, procurement routes, specifications and site supervision elements will be explored.
